Because our services are so specialized in Integrative Medicine, as providers, we do not follow patients in the hospital. We recommend that you have an ongoing relationship with a general practitioner outside of this office who is available for emergencies, urgent care and hospital admission.
We do provide emergency phone coverage to our patients during evenings and weekends but ONLY on issues that we treat. For these types of rare emergencies you may call:

Except for issues of medication reactions for medications we have prescribed, these phone calls will be charged and billed to you as phone consults.
If you need refills on any prescriptions, please speak with your pharmacy. Then, please contact the office only during the week day business hours for any issues regarding prescriptions. We do not address issues regarding prescriptions on weekends.

Please call or e-mail our office to talk with us about your specific needs. Most often, the length of the first new patient visit is 1 hour and 15 min to cover all the current health issues you are facing, review past medical history, past labs, and create with you a treatment plan and actions.
On the day of your initial appointment, it is helpful for you to arrive 10 minutes before your appointment time so that all necessary paperwork can be completed efficiently. You may choose to bring someone with you to take notes during your visit or you may bring a recording device to record the visit exchange.

Cancellations and Rescheduling
There will be no charge for cancellations made at least 48 hours before the time of appointment. Otherwise, you will be charged for half of the fee unless we are able to fill your appointment time.
Appointments scheduled for Monday must be cancelled by noon on the preceding Friday.
After your initial appointment you may choose to continue to see Jane Kennedy in person for any follow-up visit. Sometimes she will allow those follow-up consults by phone – depending upon what she feels is necessary for your best care. These are charged at the same rate as an office visit based on the time spent on the phone and any necessary time to finish caring for your needs. But, check with your insurance company, for often they do not reimburse charges for telephone consultations.
Most often return office visits are scheduled for 45 minutes with a minimum of one half hour, or longer as indicated by Jane Kennedy, NP, or according to your needs.
New Patient Forms
We are making arrangements for you to have access to electronic versions of the new patient forms so that you can fill them out on your computer at home. We will provide the details shortly.
Please remember to bring all completed forms with you to your appointment.
We are a cash based practice only and all services provided are expected to be paid for in full, at the time of service. This includes Medicare patients.
When you check out and make your payment to us, you will receive a receipt with required codes which you may submit to your insurance company for your own re-imbursement. This does NOT include Medicare and Medicare Supplemental policies since we do not participate with Medicare. Medicare patients will need to sign an agreement of understanding legally required by Medicare.
Please understand that we offer many innovative treatments and your insurance company may not pay for all of the treatments you receive in our office.
If you have questions regarding insurance reimbursement, please ask our staff before scheduling your appointment or receiving the service.
When information is requested by your insurance company, we charge for any time involved in researching and sending those reports.
If you need refills on supplements from our office, you can drop by to pick them up during office hours or you can send an email to Supplements@HormoneBalanceCal.com to place an order and have them shipped to you. If you order by email, please be sure that you have your credit card information on file with our office and that you include your phone number and shipping address. Orders received by noon on Monday will be shipped Tuesday, and orders received by noon on Thursday will be shipped Friday.
Any mailing will include a shipping and handling charge based on the actual cost of shipping plus approximately $3.00 packaging/handling fee.
Supplements may be returned to this office for credit only under special circumstances approved of by our office.
Results for lab tests requested by our office are usually received within 2- 3 weeks from the time the lab receives the specimen, with rare exceptions. (Some specialty labs take 45 days for results.) We ask that you schedule a lab follow-up appointment with Jane Kennedy, NP at least three (3) weeks after your tests.
